What will a day in the life of a Service Coordinator entail?

  • This role manages a team of staff and volunteers who support people who are ready to be discharged from local Hospitals by transporting them home and settling them safely back into their own surroundings.

  • You will meet with our service users and assess their needs, guiding or signposting them to connect with groups or to manage a health condition. You'll enable them to lead independent lives and improve their wellbeing.

  • You will liaise with partnership agencies to promote the service and build meaningful links.

  • You will produce and maintain monthly reports.

    Do you have strong leadership skills and a knack to get the best out of people?

  • Are you a good communicator?

  • Do you have knowledge of Health and Social Care services?

  • Do you have a manual UK driving licence, held for minimum of two years, and access to a vehicle? (mileage will be paid).
