Full Job Description

The Long Term Conditions (LTC) and Population Health (PH) Programme Manager will be responsible for operationalising plans for LTCs and population health management (PHM).

Working in partnership the LTC and PH Programme Manager will support decision making and strategic planning across organisational boundaries acting as a vehicle for improvement.

The LTC and PH Programme Manager will support and drive transformation as well as securing value for money in planning, commissioning and service quality. Effective project and change management expertise will be required to ensure transformation of services to improve access to services for patients, relatives and carers. The post-holder will manage and implement the SNEE ICS PHM Programme within each Alliance, improving outcomes for the population.

The Strategic Programmes team works across SNEE ICB footprint which includes Ipswich & East Suffolk, West Suffolk and North East Essex alliances ensuring transformational developments are in place across the Integrated Care System (ICS) footprint.

Please note, due to the ongoing cost reduction programme and restructure underway at SNEE ICB (due to end in Summer 2024), prior consideration for this role may be given to existing staff who are affected by this programme and this role may be removed from the recruitment process at any time., SNEE ICB has a five-year joint forward plan the Long-Term Conditions and Population Health Programme Manager will be responsible for operationalising the plan for long term conditions and population health management. The Long-Term Conditions and Population Health Programme Manager will support local projects and drive transformation as well as securing value for money in planning, commissioning, and service quality.

Manage and implement the SNEE ICS Population Health Management Programme within each Alliance, developing and embedding the use of linked data, predictive analytics and partnership working in developing models of care and interventions to improve outcomes for our most vulnerable groups and improve health outcomes for the population.

Work with Analytical teams to analyse and interrogate data for all programs of work; work with transformation and alliance leads to prioritise; work with health and care practitioners, multi-disciplinary teams, and voluntary sector workers to design and deliver the interventions for PHM and LTC area.
