In partnership with Mid Cheshire Hospitals NHS Foundation Trust (MCHFT), Pathways CIC are recruiting a WTE Social Prescribing Link Worker, who will work in collaboration with the Hospital Trust and their Workforce Management Team, to provide personalised support to the Trusts employees, to support them to improve their health and wellbeing through effective Social Prescription.
