Its core activities and associated projects are a keystone for local community engagement and development. We are looking to recruit a Social Prescribing Advisor as part of an award-winning service.

The Rotherham Social Prescribing Service works with GP Practices, Mental Health teams and Hospitals to support individuals to tackle barriers that prevent them from self-managing their health and wellbeing and becoming connected to their local community.

The Social Prescribing Advisor - Children and Young People will work with identified mental health and educational professionals; working in schools and the community to provide personalised support to Children and Young People, their families and carers. You will help them to take control of their health and wellbeing and improve their health outcomes. The role will provide active signposting to access support from the Voluntary and Community Sector (VCS) and statutory organisations in order to meet health and wellbeing needs.

  • Be able to build positive relationships and work in partnership with a range of organisations and individuals

  • Have experience of working with Children, Young People and Families, particularly with individuals living in areas of deprivation and health inequality

  • Have an understanding of issues around mild to moderate mental health needs in Children and Young People.

  • Be resilient, creative and able to respond positively to change

  • Have an understanding of the impact that social, economic and environmental factors have on health and wellbeing

  • Have excellent interpersonal skills

  • Work effectively as part of a team

  • Be able to write effective case notes and use data monitoring systems effectively

  • Understand the need for robust Information Governance including Data Protection and Confidentiality

  • Some evening and weekend work will be required to fulfill the nature of the role. Due to the nature of the role, the successful applicant will be subject to an Enhanced Disclosure and Barring Service check.
