Full Job Description

An exciting and rewarding opportunity has arisen within the Central Lancashire SIAS team. The role offers a diverse and varied workload, operating within a busy team. This role will include working in a person centred and strength-based manner. The post holder duties include include screening and triaging referrals into the service, responding to urgent requests for support, undertaking initial assessments, signposting, giving information and advice and creating support plans.

The successful candidate will be an expert in identifying the right level of care at the right time and have the ability to use their professional judgment and initiative to ensure individuals are provided with the tools to achieve their optimum level of independence.
