Practice Teacher / Clinical Team Leader

To support the Operations Manager in delivery of this wonderful service and to adopt a supportive culture for teamwork and building. The Intermediate Care Team cares for patients needing rehabilitative/social care within Community Rehabilitation Units throughout Hull and East Riding and through our UCR 2-Day service seeing patients within their own home and preventing hospital admission.

The post holder will be welcomed into a substantial and supportive rehabilitation team. We are looking for a highly motivated and passionate band 7 nurse who can provide clinical expertise in the delivery of nursing care and leadership within the team., Previous rehabilitation experience would be an advantage.
