Full Job Description

We are seeking a dedicated Exercise Referral Co-ordinator to join be part of our team. The successful candidate will play a vital role in supporting, developing, and managing referral-based exercise programs aimed at enhancing the health outcomes of our community and wider North Northamptonshire.

This role will involve collaborating with local partners, overseeing program delivery, and fostering relationships to drive positive impact.

The post holder will also be expected to deliver these sessions as and when demand requires.,

  • Support, develop, and manage referral-based exercise programs including Cardiac Rehabilitation, Cancer Rehabilitation, Pulmonary Rehabilitation, Activity on Referral, Weight Management, and other relevant projects.

  • Establish new partnerships and nurture existing relationships with local partners such as Kettering General Hospital, Northampton General Hospital, Public Health, Northamptonshire Sport and Community Health Professionals.

  • Collaborate with partners to identify opportunities for program enhancement and expansion, ensuring alignment with community health needs.

  • Oversee the daily coordination and delivery of exercise programs, ensuring smooth operation and adherence to program guidelines.

  • Provide guidance and support to program participants, addressing their individual needs and goals.

  • Monitor program effectiveness and outcomes, collecting and analyzing data to inform program improvements.

  • Stay updated on industry trends, research, and best practices in exercise referral and community health promotion.

    You must have a Level 2 Gym Instructor Qualification.

  • You must have a Level 3 Personal Training Qualification.

  • You must have an Exercise on Referral Qualification.

  • Experience in delivering Group Exercise Classes.

  • You must have one or more of the following Level 4 Qualification in Cardiac Rehabilitation, Cancer Rehabilitation or Pulmonary Rehabilitation.

  • Previous experience in coordinating and delivering exercise referral programs.

  • Strong understanding of exercise physiology, health behaviour change, and chronic disease management.

  • Excellent communication and interpersonal skills, with the ability to build and maintain effective partnerships.

  • Proven organisational and project management abilities, with a keen attention to detail.

  • Produce reports, evaluate and analyse data and present findings both internally and to external partners.

  • Ability to work collaboratively in a multidisciplinary team environment.
