Full Job Description

The post holder will be the Operational Lead for the Complex Case Management Team within the Integrated Discharge Service. They will also provide operational Business Support within this remit to the Community and Intermediate Care Division. Responsible for the Management of the Complex Case Management administrative team always ensuring support for the Service Delivery Manager: providing supervision, training, and support to the team. Taking the Lead on the essential data in relation to reducing length of stay and delays in discharge. Overseeing the management of the Complex Case 'Trigger List' and providing daily escalation to all partners to find solutions to timely discharge. Responsible for creation of Business Plans, Business Continuity Plans and Business Cases as required. Supporting across the IDS with Human Resources, Finance, Quality and Safety, Performance. Promoting Quality Improvement and leading on service improvement projects as required. Leading on specific projects,
audits and work streams as required. Working closely with Business Managers, Team Leaders and Commissioning colleagues on projects and pathway development., To be the Operational Lead who oversees the administrative function of IDS with accountability under the supervision of the Service Delivery Manager and the Head of Complex Case Management for all relevant systems and processes that support safe, personal, and effective discharge pathways for our patients. To be the service lead for the micro-management of the case load recorded on the 'Trigger List'. To be a key point of contact for the IDS for all relevant partners including patients and carers. Manage day to day running of the Complex Case Management administrative team to ensure they work effectively and efficiently in accordance with their job descriptions to provide appropriate support for the Integrated Discharge Service. Management of sickness absence, performance and annual leave of the above team including the undertaking of appropriate 1:1 meeting, Return to Work interviews and appraisals. Be conversant with the Electronic Staff Record system to ensure that all records
pertaining to employment are current and accurate, making amendments on the system in a timely manner to avoid payment errors. Be conversant with E-Roster to manage staff attendance and ensure the accurate and timely submission of information to inform payroll. Be an authorised signatory for financial transactions including travel expenses, stock and non-stock requisitions and purchase orders., Work within and ensure implementation of the policies and procedures of ELHT. To maintain confidentiality in all aspects of duty including patient information and staff personal data. Ensure the Code of Conduct for NHS Managers is always followed. Contribute to the creation and amendment pf policy, procedure and guidance as required. Maintain personal knowledge of NHS initiatives and share this information as appropriate.

In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary.
