Full Job Description

As a member of Liverpool Carers Centre team, you will work with unpaid carers to support them to access practical and emotional support.

As a Carers Development Worker, you will aid in empowering carers to take control of their own health and wellbeing. You will give time and focus on the what matters to me philosophy and take a holistic approach to connect carers to community groups and statutory services.

CORE RESPONSIBILITIES OF THE ROLE WILL BE:

SCOPE OF ROLE:

To support carers through the Carers Assessment process. This will be achieved in a variety of ways e.g., visiting carers in their own homes when appropriate or elsewhere to offer support. The role is based predominantly in the community with home visits being the primary method of contact.

o Completing Carers Support Plans with carers using a person-centred approach.

o Providing Carers with information as to the range of services available for carers and how to access them.

o Completing and submitting carers support plans in a timely fashion to ensure departments KPIs are achieved.

o Liaising with colleagues in statutory services.

o You will be an energetic and solution focused worker who will support carers to access the most appropriate services to support them in their caring role.

o Working throughout the City you will develop relationships that help and support unpaid carers while being a main enabler for holistic solutions.

ACCOUNTABLE FOR:

o Developing and sustainment of links with partnership organisations to encourage collaborative working to support unpaid carers.

o To provide advice, guidance, and practical support in the implementation of support packages.

o To support the collation, analysis, and reporting on the opportunities for organisational learning on Carers project.

RESPONSIBILITIES:

o To work collaboratively with key stake holders to raise awareness of carers known to them.

o To promote the referral pathway for carers.

o To support carers to access the relevant support dependant on their need.

o To take part in presentations, attend meetings and provided information at events and activities that will offer support to carers.

ACCOUNTABILITIES:

o Adhere to data protection legislation and data sharing agreements, ensure that all carers that are supported by the service are accurately recorded using agreed templates, following up to ensure that they have received good support; completing monitoring as required, to demonstrate outcomes of the service.

SUCCESS MEASURES:

o Delivery of business objectives Carers Project and carers supported.

o Achievement of related targets.

o Adoption of best practice.

o Provision of reliable and accurate information and data, including KPIs.

o Increasing visibility and information.

o Expanding business.

o Meeting any internal or external demands for information to demonstrate. compliance with targets, standards, statutory, regulatory, or other requirements.

o Maintain Local Solutions profile by providing excellent customer focused support when dealing with all internal and external parties.

o Experience of working in health or social care.

o Experience of supporting unpaid carers or working with people.

o Experience of working as part of a team but the ability to prioritise own workload, managing own diary, building relationships with both carers and professionals, management databases and inputting information.

o Attendance at occasional evening or weekend events.

o Technical expertise should include ability to use Microsoft office, Outlook.

o The successful candidate will require an enhanced Disclosure & Barring Check and undertake mandatory safeguarding training as appropriate for the role.
o Travel is required so a car owner/driver is ESSENTIAL as this is a city-wide post
