Supported Care Service Lead Southern CCG Area

£45,840 - £50,613 per annum
Based in Wymondham

The Supported Care Service is a multiagency admission prevention service working across North and South Norfolk. The service aims to support people with Long Term Conditions to keep them at home and help them to maintain their independence for as long as possible. Where an admission has already occurred the service aims to offer discharge support. This admission prevention service includes triage, assessment, care plan setting and provision of support at home. The team includes colleagues from Norfolk First Support, Norfolk

Community Health & Care NHS Trust, Norfolk Carers and Community Action Norfolk.

Then this service could be for you! You will need the ability to co-operate and collaborate, work within new processes and pathways across a multi-agency workforce in order to build an integrated approach to working that is mutually respectful of all disciplines involved.

The post holder will provide overall operational leadership, management and co-ordination across the South Norfolk Supported Care Service.
