Social Worker 49417 SSRE

Norfolk and Norwich University Hospital

£32,029 - £33,799 per annum

37 hours per week (7 day working will be expected as part of the role)

Fixed term contract/secondment opportunity for 2 years

We are a supportive and friendly team working with clients from 18yrs onwards based at the Norfolk and Norwich University Hospital in Norwich. Right now, we are looking for an experienced social worker who is passionate about making a difference to join our dedicated team.

With a flexible base, either in a hospital and/or community setting, you will enjoy working in partnership with a wide range of health professionals in a time challenged and fast-paced environment. We are proud of our multi-disciplinary working environment and our positive working relationships with our Health colleagues.

You will manage a case load working closely with colleagues and partners in the assessment process to achieve timely discharges in line with the Care Act. As an experienced social worker, you will undertake a range of assessments such as Care Act Assessments; Discharge to Assess Process; Mental Capacity Assessments; and will lead on Safeguarding enquiries.

It is a great time to join us as we embrace our living well agenda to support people to be independent, resilient and well. Were investing in social care and changing the conversation for social work to provide better outcomes, building on the strengths of individuals.

The team provides a 7 day a week service and you would be expected to work occasional weekends.

We are committed to facilitating a learning environment and encourage opportunities for on-going professional development including supervising students and supporting social workers to become practice educators.

It is essential that you have a professional Social Work qualification (MA or BA Social Work, DipSW or CQSW) and you are registered with HCPC.
