Norfolk First Support are working in partnership with the Clinical Commissioning Groups to develop a new Supported Care model, which is about making adjustments to the way services are organised so that we reach certain patients before they reach crisis point. The programme aims to keep people at home and help to maintain independence as long as possible. It is part of an on-going process to establish local joined up community provision, connecting health and social care with the voluntary sector and wider community.

Work across health and social care and in partnership with patients and their families and carers, the voluntary sector and the wider community.

We are looking for the following people to join our Norfolk First Support team on this new exciting venture. We do require a commitment that you will work an alternate weekend.

Reablement Support Workers - Supported Care Nights

All posts will be based in Wymondham or North Walsham
Hours range from 21 to 37 per week, with various shift patterns available
Salary ranges from £9.36 £12.93 per hour depending on role
