Pathways Support Worker No Second Night Out

Full time- 38 hrs a week

Starting salary £19,945 

Were looking for a forward-thinking individuals who are experienced in delivering person-centred support to vulnerable people to join the Pathways initiative and help clients with entrenched multiple needs reach their personal goals by providing routes out of homelessness.

This role will have direct responsibility for the assessment and support of people who are homeless or at risk of homelessness and liaise with all persons identified as appropriate to the support of each individual.

 

 

Youll need recent experience of providing support services to people with multiple and complex needs, with knowledge of Norwich based services for the client group. Calm, well organised and flexible in approach, youre willing to contribute new ideas, methods and processes too, in order to improve service delivery. Proficiency using Microsoft Office applications and case management systems is also required.
