Full Job Description

We are seeking an enthusiastic Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward! We are recruiting one full time position.

Virtual Wards deliver hospital-level care at home using remote monitoring, digital tools and multi-disciplinary community support. Our service is part of a system-wide collaborative across Nottinghamshire Healthcare, NUH, Sherwood Forest Hospitals, Doncaster & Bassetlaw Teaching Hospitals and CityCare, ensuring seamless care across acute and community settings.

We support pathways including frailty, respiratory and admission avoidance. As a Band 6 Senior Community Practitioner, you will provide timely assessments, person-centred care and advanced clinical decision-making in patients' homes, working both autonomously and within an established MDT.

This role offers opportunities to develop advanced assessment skills, contribute to innovative virtual care models and play a key part in system transformation. We welcome applications from nurses passionate about acute community care, digital innovation and supporting people to recover safely at home.

Main duties of the job

Deliver timely, person-centred assessments and provide high-quality clinical care to patients on the Virtual Ward for up to 14 days.

Work autonomously and within the MDT (including Matrons, Nurses, AHPs and support staff) to prevent unnecessary hospital admissions and support safe early discharge.

Interpret remote monitoring data, undertake face-to-face clinical reviews, identify deterioration and initiate appropriate interventions or escalation.

Lead and coordinate safe, effective discharge planning and contribute to daily MDT discussions on treatment and risk.

Support junior staff through supervision, role-modelling and clinical guidance.

Maintain accurate documentation on SystmOne and uphold professional standards, governance, and patient safety.

Work flexibly across respiratory, frailty and admission avoidance pathways, following lone-worker and dynamic risk assessment procedures.

#TeamNottsHC comprises over 11,000 dedicated colleagues who #MakeADifference every day. We deliver intellectual disability, mental health, community health, forensic, and offender healthcare services across Nottinghamshire, Leicestershire, Lincolnshire, and South Yorkshire. Our care is provided from over 200 sites, spanning community locations, acute settings, and secure environments, including prisons.

We are one of the largest mental health and community Trusts in the East Midlands and one of Nottinghamshire's biggest employers. We also host national and regional services, such as the National High Secure Deaf Service and the Nottingham Centre for Transgender Health.

We offer a variety of employee-led staff networks, including Equality, Diversity, and Inclusion (EDI) groups, the Green Champions network, the Freedom to Speak Up network, the Health and Wellbeing Champions network, and the Menopause Champions network. These networks play a vital role in supporting our diverse workforce and promoting a culture of inclusivity.

The health and wellbeing of colleagues is a top priority. We invest significantly in this through our in-house occupational health and staff counselling services, supported by a dedicated Health and Wellbeing team.

The Trust is committed to reducing its carbon emissions, with a specialised Energy and Environmental team working to ensure compliance with environmental legislation, enhance our environmental performance, and achieve our net-zero commitment.
