Full Job Description

The Urology Cancer Support Worker will work closely with cancer speciality clinical teams at Wye Valley NHS Trust. Specifically the post involves supporting clinical teams identifying patients following a cancer diagnosis are offered a Holistic Needs Assessment (HNA) and provided support with their Health & Wellbeing, both face to face, via telephone or in group sessions.,

  • Identify all patients who have recently received a cancer diagnosis and offer a Holistic Needs Assessment.
  • Offer holistic support to patients and family and friends.
  • Contribute in the delivery of the HOPE course (Help to Overcome Problems Effectively), designed to support to help patients who have had/ or have cancer to get on with their lives.
  • Signpost and refer patients to suitable services both internal and external to help address patient's holistic concerns.
