Reablement Team provides a short-term service designed to help people develop the confidence and skills they need to live as independently as they can.
We are a successful team who have been able to ensure that 80% of our service users have remained independent in their own home without any formal support. We are also part of a team who were highly commended by the Health Service Journal, for improving value in the care of frail older patients and we were part of a wide team highly commended for Integrated Discharge Planning and Reablement.

Your next role
The Reablement Support Worker role is part of an integrated Reablement Team and the wider Adult Health & Social Care multidisciplinary teams, to provide support with personal care and reablement to service users in their own homes and community through outcome focussed support. This is achieved by supporting individuals to regain, improve and maintain skills of daily living, promoting maximum independence.
As a Support worker, you will support service users by assess service users' needs and monitor against aims, objectives and outcomes as part of their individual care plan, liaise with other professionals and the wider team by providing feedback and participate in the monitoring of service users mental and physical health.
In addition, you will assist service users with bills, shopping and prescriptions and contribute to the promotion of positive links with local community agencies/groups. For very vulnerable service users, you will assist them in developing, regaining and maintaining skills to maximise their independence, this will include very personal care tasks, whilst always treating them with dignity and respect.
You will maintain accurate and up to date records and report any accidents or concerns. You must be able to liaise with other professionals/family members and other team members, reporting any key concerns to respective senior staff in line with the requirements of all relevant legislation and local policies, procedures and guidance, including potential safeguarding concerns.
The successful candidate must be highly motivated and enthusiastic about providing a first-class service to the residents of Swindon. Be able to work independently, using initiative and as part of the wider Reablement Team.
You will need to have experience and knowledge of caring for vulnerable adults with a wide range of complex needs, including personal care and have experience of working in the community or care home.
The ability to travel throughout the Borough is essential and you will need a full driving licence and access to a vehicle whilst working.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act "for the effective performance of a customer-facing role".
