Full Job Description
We are seeking a compassionate and motivated Support Worker to join our specialist Children and Young People's Eating Disorders Service. This rewarding role involves providing therapeutic support and spending meaningful time with children and young people experiencing eating disorders, helping to promote recovery and emotional wellbeing. You will be responsible for delivering planned, recovery-focused care to a defined caseload of children and young people with eating disorders, ensuring support is tailored to individual needs. You will work collaboratively with colleagues across Trust services to provide seamless care along agreed pathways and liaise effectively with external partners, including statutory, independent, voluntary, and charitable organisations, to support access to appropriate services. You will support service users to maintain their place within the community and assist with transitions between services where required. Working closely with the Lead Professional, you will contribute to the assessment, planning, implementation, and evaluation of individualised care plans. You will also play a key role in supporting young people and their families, including providing practical and emotional support during mealtimes. - Input patient related data into appropriate clinical system such as Rio.
- Observe and record data using appropriate methods, tools and technology, including performing simple audits or surveys and assisting with occasional clinical trials or research projects. Decisions and Judgements
- Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues
- Works with other Trust services to promote seamless service-user focused care, transfer and discharge on identified care pathways including accepting routine referrals during the day.
- Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
- Regularly supports service-users to access a range of activities and services to maintain their independence and promote their recovery.
- Regularly provides advice and guidance - promoting healthy activities and lifestyle choices. Communication and Relationships
- Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
- Relates appropriately with service-users, carers, colleagues and others - at all times
