Full Job Description
An exciting opportunity has arisen for a band 6 role (RNLD/RMN), to join the community-based Learning Disability Intensive Support Team (LD-IST), based at Townend Court. The Intensive Support Team embraces the national context of 'Transforming Care' and the vision set out in "Building the right support" and the "National service model for commissioners of health and care services". Successful candidates will work as part of a multi-disciplinary team, offering clinical support to individuals with complex health needs, their families and care providers by implementing person-cantered care plans in collaboration with the multidisciplinary team including Nursing, Speech & Language Therapy, Occupational Therapy & Psychology . The successful candidate will work across the (IST) and the Learning Disability inpatient Unit. This role provides the chance to develop skills and experience in two closely linked services whilst ensuring that continuity of care for patients as they move through the Learning Disability Pathway. Your experience will be vital in maximising a positive patient journey, primarily preventing admissions in the community however also, should the need arise, supporting the admission, treatment and subsequent discharge. Our valued team members support some of the most vulnerable people in society, demonstrating care and compassion and treating people as individuals. The successful applicant will work a pattern of 0800-2015 over a 7-day period.,
- To provide therapeutic interventions within the community for clients with a learning disability and behaviours of concern.
- Provide person centred care, supporting clients and their families/careers to be involved in their own care planning.
- Provide consultation to other divisions and servicers for clients under the care of the team.
- Take part in service development as appropriate and support the IST leadership in a lead role beneficial for the service.
- Provide mentorship to students and supervision to other staff within the team.
- Charing of meetings e.g. team meeting, CPA reviews
