Full Job Description
You will be joining KMPT at an exciting time. The work around the implementation of the Community Health Framework for Mental Health will enable Social Work to develop a strong profile within secondary care mental health service to improve patient experience and compliment our existing MDT. As a social worker you will provide a high standard of support and care delivery with regards to social care needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems. You will be working in the Medway and Swale HTT/RR The Team covering the Medway and Swale part of Kent., You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion and social justice. The role will entail assessment, support planning and the opportunity to provide 1-1 strength based therapeutic interventions. You will act as Lead Health Care Professional, (where applicable) with individuals who have serious and enduring mental health difficulties. The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
