    Salary ranges from £44,075 for Level 2 and from £48,226 for Level 3 depending upon the level of experience you may have.

What You'll Do: You will support people living with long-term conditions to live well in the community. You will do this by working alongside them, their carers' and their families to understand what matters most, and what support is needed to help them live a safe and independent life. Flexibility: We have opportunities for full-time, part-time and flexible working. We have an agile hybrid working model with an element of home working, and flexible office spaces., We can help with the re-registration process for those returning to social work. You must also hold a valid full UK driving license, with access to your own car, or be able to travel efficiently (and sustainably where possible) to fulfil the needs of the role using public or other forms of transport.
