Full Job Description

The Annual Review team will undertake annual reviews for individuals aged 18 years and above who have identified eligible needs as defined in The Care Act (2014) and have an associated support plan. They need to be residing within the city of Stoke on Trent or are funded in a provision outside of the city. The reviews will be a combination of face to face reviews and where appropriate these can be telephone reviews.

The team operates across all client groups including functional mental health, learning disability, physical disability and older people including transitions. The team do not accept direct referrals, and work is planned based upon forthcoming review dates and is task focused.

The team will link in and arrange a transfer to front line operational teams if following a review a longer-term piece of work is required.

Where cases are allocated to individual workers and safeguarding concerns are identified via the front door this work should be passed to the appropriate operational team. The workers attached to the annual review team will progress any safeguarding concerns identified as part of their review to the point of decision making. If a safeguarding enquiry is required, this may be transferred to the appropriate operational team., ·Conducting reviews within the framework of both The Care Act (2014) as well as Section 117 Reviews (The Mental Health Act)

·Entering the review onto the social care electronic recording system

·Updating individual support plans

·Liaising with other professionals and service providers

·Must be a registered Social Worker with Social Work England

·Given some reviews may be out of the area, a car driver is preferred or a willingness to use public transport
