Are you committed to delivering compassionate, person-centred care that empowers individuals to remain independent? Join our dynamic Neighbourhood Rehabilitation Team as a Rehabilitation Support Worker (RSW) & make a real difference in your community

Youll work under the guidance of registered healthcare professionals to deliver high quality, person-centred care on the Intermediate Care pathway. Youll work both independently & collaboratively supporting individuals in their own homes & care homes

You will promote independence through mobility practice & encouraging participation in physical exercise programmes & therapy plans.

You will also assist with meal preparation and the development of daily living skills, while delivering delegated care tasks such as personal care and medication support.

Youll have a desire to work in the NHS, be motivated & flexible, & work a shift pattern including weekends & bank holidays

This is a community role, a current driving licence & access to a vehicle to undertake travel in connection with the duties of the post is essential, As Rehabilitation Support Worker you will

Provide high standards of safe & compassionate care that maximises patients rehabilitation potential

Work in close liaison with the MDT, excellent communication & time management skills are essential

Assist in initial assessment & development/implementation of care plans

Participate in training, education, & clinical supervision to develop skills & knowledge & apply continuous reflection on own practice

Have ability to work independently and as part of a team

Demonstrate strong communication and observational skills

Maintain accurate clinical records

Support safe use of equipment such as hoists, walking aids & pressure cushions
