Are you passionate about making a real difference in people's lives? We are looking for a dedicated Rehabilitation Assistant to join our acclaimed Tri-Borough Falls Prevention Service within CLCH, supporting residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who are at risk of falls. As a valued member of our team, you will work closely with Physiotherapists, Occupational Therapists and fellow Rehabilitation Assistants, with guidance from your Team Lead and Clinical Lead. You will play a pivotal role in delivering strength and balance classes, while also working independently in clinic settings to teach exercises, monitor memory, balance, fear of falling and measure blood pressure. This role also offers the rewarding opportunity to work directly in clients' homes,helping them improve mobility, balance, function and safe use of equipment under therapist guidance. Comprehensive training on outcome measures and screening tools will be provided, enabling you to conduct assessments confidently and independently in both community and clinic settings. - Provide both general and specialist support to patients under the guidance of a registered allied health professional.
- Work across a variety of settings, including community centres, clinics and patients' homes.
- Develop competence through training and competency assessments,promoting individualised, client-centred care.
- Support patients in achieving optimum independence and function, ensuring a positive and inclusive care experience.
- Assist the Falls Prevention Team in achieving key performance indicators while maintaining high-quality, safe and accessible services.
