An exciting opportunity has arisen for an enthusiastic and self-motivated individual to join the North Integrated Discharge Hub based at Royal Stoke Hospital

The role of the Rapid Discharge Facilitator will be based in our Triage hub, providing hospital discharge guidance and support to our referring acute hospital trust.

The successful candidate will be part of a triage team that receive referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients' needs as part of a multi-disciplinary team with a key role in decision making., The post holder will liaise with a variety of people (service users, colleagues, member of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual's ability/potential for reablement via the discharge to assess pathways alongside supporting admission avoidance assessments to prevent a hospital admission.

The post holder will be working autonomously as part of a hub and spoke model and will be expected to be able to use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams.
