Full Job Description
We have an exciting opportunity to join a supportive and dynamic practice to develop your career as a Practice Nurse. We have hours available for a full time position or a job share. Please contact us with any questions regarding hours and days as we would consider part time and compressed hours., As our Practice Nurse you would be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
The practice will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.
We have a list size of 6550 and a full team including GPs, Nurses, HCA, ACPs, Clinical Pharmacists, First contact physio, Mental Health Practitioner, Management team, Dispensers, Reception, Secretarial and Admin staff. We are an approved training practice and have GP Registrars working in our team.

Salary offer is based on experience. The offer will also be adjusted commensurate with extra skills the candidate could contribute.
