Full Job Description

Are you an experienced Registered Nurse looking to join a friendly and motivated Out of Hours (OOH) Community team? We are a supportive multi-disciplinary team who are passionate about delivering high quality care to housebound patients, in partnership with primary care and secondary care, covering moor-to-sea, coastal and Torbay districts. Joining our team as Band 5 Registered Nurse, you will provide responsive, planned and unplanned support to patients requiring urgent nursing within their own homes and residential care settings. We work in small teams of two which includes yourself and one Healthcare Support Worker (HCSW) where you will work in partnership throughout your shift, this will include shared driving. Whilst working for our Out of Hours team you will have access to Trust provided Pool Cars for all of your work-transport needs to ensure you can deliver planned and unplanned care. We believe in a healthy work life balance, should you be successful your full times hours will be 33 hours per week over 3 nights from 7am - 7pm, with 4 rest days. You will have regularly 1-2-1s scheduled to discuss you career progressions, off duty requirements, service improvements and safe space to raise concerns.,

  • Working in partnership with HCSW to support patients and their carers
  • Treating Post non-injury or minor injury falls which can include wound dressings and supporting patients with catheterisation, bowel care and treatment escalation plans.
  • Responsible for Community Nurse Equipment i.e., Scales, Bladder Scanner, Doppler
  • Guide and mentor junior staff and students
  • Escalate any concerns to team leaders or Community Nurse Lead
  • Generate prescriptions and order pressure relieving equipment using available IT systems in place.
