Full Job Description

As an Occupational Therapist you will be required to:

· To undertake Occupational Therapy assessments for a designated caseload addressing Occupational performance and skills deficits, enabling the patient in areas of Self-care, Productivity and Leisure.

· To work with patients to identify Occupational Therapy goals as part of overall treatment plan.

· To plan and implement individual and group interventions in collaboration with the client group, using graded activity to achieve therapeutic goals.

· To monitor and evaluate occupational therapy treatment in order to measure progress and assure effectiveness of the intervention.

· To participate in the planning, evaluation and audit of practice, clinical pathways and protocols within your area.

· To participate in the delivery of the Occupational Therapy Strategy., The post holder will be responsible for the planning and delivery of Occupational Therapy within the designated area. Manage a caseload of service users with complex needs, using evidence based/person centred principles to assess, plan, implement and evaluate interventions.

  • Current relevant professional registration (HCPC Registration)

  • Experience working with elderly patients

  • Previous intermediate care experience and NHS experience

  • Enhanced DBS

  • Car driver / owner able to travel to other locations within the trust using own vehicle
