Full Job Description
The post holder will deliver high-quality, patient-centred care through advanced clinical assessment, diagnosis, and management of acute and chronic conditions. Key responsibilities include:
- Independent assessment, diagnosis, and treatment of patients
- Prescribing and medication reviews in line with current guidelines
- Managing same-day urgent and routine appointments
- Working collaboratively with GPs, nurses, and the wider multidisciplinary team
- Contributing to service development, quality improvement, and clinical governance, The post holder will work as an autonomous clinician within the GP practice, providing advanced clinical care to patients presenting with acute, chronic, and complex health needs. The role requires independent prescribing, advanced clinical assessment skills, and the ability to manage patients across the lifespan within a primary care setting. The ANP/ECP will work collaboratively with GPs, nurses, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care, contributing to service development and quality improvement within the practice. Main Duties and Responsibilities Clinical Responsibilities
- Undertake comprehensive clinical assessments, including history taking, physical examination, and diagnosis
- Manage undifferentiated and differentiated presentations independently
- Assess, diagnose, treat, and discharge or refer patients as appropriate
- Prescribe medications independently in line with clinical guidelines and best practice
- Manage acute, urgent, and long-term conditions
- Order, interpret, and act upon diagnostic tests and investigations
- Provide health promotion, disease prevention, and patient education Care Management
- Provide same-day urgent care appointments and routine clinical appointments as required
- Refer patients to secondary care, community services, or allied health professionals where appropriate
- Ensure continuity of care through appropriate follow-up and safety-netting
- Maintain accurate, clear, and contemporaneous clinical records Multidisciplinary Working
- Work collaboratively with GPs, practice nurses, HCAs, pharmacists, and the wider PCN team
- Participate in MDT meetings and clinical discussions

- Participate in clinical audit, quality improvement, and service development initiatives
