Full Job Description
An exciting opportunity has arisen for a dynamic Mental Health Nurse to join the Positive Lifestyle Team on a full time basis. The successful applicant will work across the Greater Preston and Chorley/South Ribble Clinical Commissioning Group footprint. The role will involve working with Service Users referred from the CMHT's requiring interventions around Physical Health Monitoring and improving their quality of life, as well as monitoring their mental health. It is anticipated that the successful applicant will have a caseload under supervision. The successful applicant will be able to work well within a team as well as independently and have excellent liaison and communication skills, they will need to be flexible and adaptable and able to form good working relationships with others.
- To work as part of a team to ensure that patients are provided with the support they require, to maintain stability in their mental health & address their physical health needs, as they step down into primary care.
- To act as a member of the Multi-Disciplinary Team, supporting the effective assessment, planning, clinical treatment, monitoring and evaluation of care (under supervision) given to service users with severe and enduring mental illness under a Case Management process.
- To provide nursing activity in the delivery of pharmacological treatments and subsequent patient monitoring for desired outcomes and unwanted side effects. Such treatments include depot medications and oral medication regimes. The work undertaken will include delivery of treatment initiation programmes, monitoring of medications and planned reductions of medications, which meets, expected national standards for this client group.
- To work in close association with the Team Manager and Senior Mental Health Practitioners, other clinical treatment practitioners, medical and pharmacy colleagues, community team managers, staff from The Acute Hospital Trusts and medical and nursing staff within primary care to deliver service cohesion and effective interfacing.
