  • Support the Home First team with customer assessments and reviews.
  • Work with allocated customers to identify and deliver high-quality, person-centred outcomes.
  • Build strong relationships with customers to achieve personalised goals and improve independence.
  • Work collaboratively with social work teams, health partners and the voluntary sector to support integrated care.
  • Make a meaningful difference to the lives of residents across Suffolk.
  • Receive regular support and professional supervision as part of a collaborative team environment., This authority is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment. DBS (Disclosure and Barring Service) checks or police vetting will be required for relevant posts. #LI-Hybrid
