Full Job Description
The postholder will maintain and continuously develop their clinical knowledge, skills, and expertise to support enhanced clinical judgement and autonomous decision making. They will independently undertake complex and holistic assessments, applying advanced clinical reasoning and evidence based practice to deliver safe, effective, and person centred care for individuals with complex needs.
The role involves critically evaluating clinical interventions, managing complex caseloads, and supporting patients from diverse cultural, social, and belief backgrounds through enhanced assessments, diagnostics, and interventions. The postholder will work both independently and as part of multidisciplinary teams across community and clinical settings, adapting to changing healthcare demands, evolving technologies, and increasingly complex patient presentations.
They will engage patients, families, and carers in the co production of care plans and wellbeing strategies, promoting self management and independence wherever appropriate. The role includes providing clinical leadership, supporting integrated health and social care working, supervising and supporting colleagues, and recognising the boundaries of professional practice.
The postholder will plan, deliver, monitor, and evaluate complex care, interpret diagnostics, contribute to differential diagnosis, arrange further investigations, and respond effectively to unpredictable clinical situations.
We are seeking a highly motivated and experienced Extended Clinical Practitioner (ECP) to join our multidisciplinary team. This is an exciting opportunity for an advanced clinician who is passionate about delivering high-quality, patient-centred care and driving excellence in clinical practice.
As an Extended Clinical Practitioner, you will provide a high standard of complex, enhanced care to patients, utilising advanced levels of clinical judgement, knowledge, and skills. Working autonomously and collaboratively within a multidisciplinary environment, you will assess, diagnose, plan, implement, and evaluate care for patients with complex healthcare needs.
You will consult closely with patients, families, carers, and healthcare professionals to undertake comprehensive assessments and develop evidence-based care plans tailored to individual needs. The role requires critical evaluation of clinical problems, application of advanced assessment and diagnostic skills, and the ability to make informed clinical decisions using the latest evidence, interventions, and technologies.
