The post-holder is responsible for the management of a defined caseload and for taking continuing responsibility for the development and delivery of programmes of care, being responsible for the comprehensive assessment, planning, provision and evaluation of patient care needs. We are seeking highly motivated SPQ District Nurses, to join the Integrated Care Community Teams across South Tyneside and Sunderland. You will be involved in the provision of high quality nursing assessments and be responsible for the delivery of clinical care. The ability to communicate positively at all levels will be an essential attribute of the post holder, as well as a flexible self-confident approach to work. The post holder will have the knowledge and experience to provide a valuable contribution to the development of more flexible and innovative working both within the team and across organisations in the statutory and non-statutory sector.
