Full Job Description

We are looking for a highly motivated and experienced Diabetes Specialist Podiatrist to join our community team, primarily based at Lisson Grove, with flexible working across Westminster, Kensington & Chelsea, and Hammersmith & Fulham. This role offers an exciting opportunity to help shape specialist podiatry services and support the development of integrated diabetic foot care across community, primary, and acute settings. The post holder will deliver expert assessment, diagnosis, and treatment for patients with complex and high-risk diabetic foot conditions, including ulceration, infection, neuropathy, and vascular disease. Working autonomously, you will manage a complex caseload while collaborating closely with multidisciplinary teams to improve patient outcomes, prevent hospital admissions, and reduce the risk of lower limb amputation. In addition, you will contribute to service development, clinical governance, research, and patient education, promoting best practice and high-quality, evidence-based care.   • Accept and triage urgent and emergency referrals for people with diabetes from GPs, community podiatrists, and other healthcare professionals, ensuring timely and appropriate specialist intervention.
  • Provide responsive, high-quality specialist care for patients with complex and high-risk diabetic foot conditions across community settings.
  • Work collaboratively with multidisciplinary teams to ensure effective care coordination and seamless patient pathways across services within CLCH
  • Contribute to the planning, development, and delivery of education and training programmes for clinical and care staff, supporting up-skilling in the management of diabetic foot conditions.
  • Deliver structured patient education to individuals with diabetes and their carers, promoting self-management, prevention strategies, and early recognition of foot complications.
  • Support the promotion ofbest practice in diabetic foot care across community and primary care settings.
