Full Job Description

Looking for an experienced registered Nurse to join the Out of Hours community Nursing team. Delivering high standards of holistic care to our patients across the Torbay and South Devon catchment area at night, preventing hospital admissions and aiding safe discharges. The B6 will be in charge of the shift and team on, allocating and prioritising workload. Managerial and supervision aspects of the job role, supporting the B7 lead. We have pool cars to drive, as we cover a large area.,

  • To communicate formally and informally with the zone DN Team Leader, to ensure best practice and consistency with the community and intermediate care service in Torbay Care Trust
  • To communicate fully and in depth with the multidisciplinary team discussing patient treatment options and progress
  • To communicate and liaise effectively with the integrated health and social care team, voluntary agencies, secondary care, G.P.'s, carers and relatives
  • To use set transfer and discharge protocols
  • To complete regular supervision sessions, both individual and group.
  • Working across the Intermediate care and community nursing teams to provide crisis response nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement to prevent hospital admissions. Aiding hospital discharges to ensure patient safety and providing specialist clinical advice to the multi-disciplinary team.
