Full Job Description
Are you passionate about improving outcomes for some of the most vulnerable patients in our healthcare system?, 1. Manage a caseload of patients experiencing homelessness 2. Complete and oversee Housing Checklists 3. Coordinate multi-agency discharge pathways 4. Escalate and resolve barriers to discharge
- Contribute to BCF reporting and performance metrics
- Support development of homeless discharge pathways at NMUH
Experience influencing across NHS and Local Authority partners - The ability to directly impact patient outcomes and hospital flow
- A supportive and forward-thinking multidisciplinary team
