Full Job Description
- Provide timely assessments for children and young people presenting with moderate to severe mental health needs, including deliberate self-harm assessments.
- Arrange appropriate follow-up appointments and ensure effective signposting to relevant services.
- Provide advice, support, and education to the wider professional network, including the Hub Team.
- Work closely with the Mindworks Alliance, School Based Needs teams, Surrey Well Being Partnership, and other partner agencies to respond to referrals.
- Conduct specialist assessments and provide treatment to children, adolescents, and their families either independently or jointly with Hub staff.
- Carry out autonomous clinical assessments for young people with complex or co-morbid mental health needs.
- Work within a multidisciplinary team (MDT) providing assessment and treatment for children and young people with complex mental health difficulties.
- Manage an individual caseload from referral through to discharge or transfer, including medium to high-risk cases.
- Complete and oversee initial assessments and facilitate warm handovers or referrals to appropriate services.
- Participate in the Hub duty rota and respond to queries from families and the wider professional network.
- Develop crisis and contingency plans with young people and families, including risk assessment and positive risk-taking approaches.
