Full Job Description

The post holder will provide specialist nursing care, advice, guidance and leadership deploying their specialist theoretical and practical knowledge to;

oAssess, plan, implement and evaluate evidenced-based nursing care.

oProvide specialist advice and excellent clinical practice.

oWork autonomously as a lone worker in the community.

oProvide specialist education to patients, their families and support new members of staff and students to develop their knowledge.

oWork collaboratively and co-operatively with clinical colleagues.

oDemonstrate the Health Board's values and ensure that others demonstrate the same values., oWork alongside the multi-disciplinary team and provide specialist nursing advice as appropriate to own areas of expertise.

oBe visible and accessible in the clinical area to the nursing and wider multidisciplinary team, patients and service users.

oMonitor and evaluate the standards of care provided.

work autonomously as a lone worker as per trust policy

Provide accurate, up-to-date, evidence-based information to patients and their families to ensure they make the best decision regarding modality.

This is a teaching post which involves travel within the regional area of South West Wales as a lone worker as per trust policy
