Full Job Description

We are looking for a motivated and compassionate Chronic Disease Management Nurse to join our patient focused and friendly team., The ideal candidate will be experienced in primary care, long term condition management and health promotion and will play a key role in delivery of high-quality care to patients with chronic and complex health conditions. They will provide advanced clinical care to patients whilst leading in chronic disease management, infection control and Quality Framework Outcomes.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.
