An exciting opportunity has arisen to appoint a Band 6 health care professional to join the Rapid Access Acute Rehabilitation service in a Case Manager Role.

This post will be based in the RAAR (Rapid Access Acute Rehabilitation) service, a specialist service providing level 1 inpatient neuro-rehabilitation at an 8 bedded unit at St George's Hospital.

The Case Manager will maintain adequate flow through the 8 bedded RAAR service by proactively liaising with external providers and agencies including but not limited to specialist rehabilitation providers, bed managers, community therapy teams, social services, housing services, equipment providers, and third sector providers.

The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs., The Case Manager will maintain adequate flow through the 8 bedded RAAR service by proactively liaising with external providers and agencies including but not limited to specialist rehabilitation providers, bed managers, community therapy teams, social services, housing services, equipment providers, and third sector providers.

The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs.

The post holder will be required to have a good knowledge of health and social care practice and be able to demonstrate an understanding of the interaction between hospital discharge and Continuing Healthcare and Funded Nursing Care, and be prepared to extend their skills, knowledge and competence in this field.
