Full Job Description

An exciting opportunity has arisen for a motivated and skilled Mental Health Nurse, Occupational Therapist or Social Worker to join our Early Intervention in Psychosis Team in Wigan as a Care Coordinator. The main aim of the Early Intervention Team is to offer treatment for individuals, aged from 14 - 65 years, who are experiencing a First Episode of Psychosis in line with NICE guidelines. We achieve this through offering :

  • Care Coordination.
  • Evidence-based group and individual psychological interventions, including Cognitive Behavioural Therapy for psychosis and Behavioural Family Therapy.
  • Treatment, support and education regarding medications.
  • Regular physical health monitoring.
  • Support with accessing employment, education and vocational activities.
  • Opportunities to engage in social activity and physical health promotion.
  • Support for families and carers.
  • With line management and clinical supervision from within the Team, the post holder will work as part of the integrated MDT, providing clinical interventions. These will focus on relapse prevention, improving quality of life and collaboratively working towards recovery. The post holder would be expected to maintain and co-ordinate timely and appropriate interventions through assessment for a caseload of service users. This includes respecting individuality, values, culture and religious diversity of service users and colleagues. Main duties of the job
  • To hold and manage a caseload of service users who havebeen identified as experiencing their first episode ofpsychosis, including needs assessments, care planning anddelivering psychosocial interventions.
  • To complete specialist assessments for referred serviceusers accepted into the service when required.
  • To undertake risk assessment and risk management forindividual service users and to provide advice to otherprofessionals on acute and chronic aspects of riskassessment and risk management.
  • To work as part of our integrated MDT, and to participate ina clinical decision making process to determine the mostappropriate management plans for all service users who areunder the care of the team.
  • To participate in the duty rota operated by the EarlyIntervention Team.

    Education / Qualifications
  • Essential
  • Registered on the appropriate part of the NMC register and hold a current NMC registration or holds a relevant professional qualification (to be maintained)
  • Desirable
  • Assessor / Mentorship Training
  • Post registration training in relevant field.
  • Experience Essential
  • Experience of working within a multidisciplinary team to support service users and their families.
  • Experience / knowledge in the application of risk assessment / management and care planning.
  • Experience in effectively managing own caseload.
  • Ability to establish good working relationships with a range of providers including GP's.
  • Experience of implementing confidentiality and keeping timely / accurate case records.
  • Evidence of participating in multi-agency clinical meetings.
  • Desirable
  • Community experience.
  • Experience of working as a Nurse in Charge / Shift Coordinator.
  • Experiencing of working with individuals with psychosis.
  • Knowledge Essential
  • Knowledge around relevant local and national guidelines / procedures / policies relating to mental health nursing care.
  • Knowledge in relation to safeguarding vulnerable adults and children.

  • Ability to undertake visits on a daily basis to service users homes within Wigan area and travel to clinical meetings at Trust and Non-Trust venues and other community venues, relating to your service user activity.
