Band 4 Associate Practitioner in Physical Health - Bristol

To carry out comprehensive physical health assessments and interventions for Community Rehabilitation service users who live in the community in Bristol., As a key member of the Bristol Community Rehabilitation Team, you will conduct comprehensive physical health assessments which include various physical health observations, phlebotomy, electrocardiogram tracings and health and wellbeing advice and referrals.

You will be a valued part of a multidisciplinary community rehabilitation mental health team. The team works with people with complex mental health problems with a focus on working with people with complex psychosis. The team provides a wide range of rehabilitation interventions including physical health assessment and interventions based on the Lester Tool.

The role involves working with people in their own homes and community settings across Bristol and requires independent and lone working.
