Are you an Advanced Paediatric Nurse Practitioner looking a join a growing social enterprise with great benefits including a £4,000 welcome/retention bonus, NHS pension and NHS car fleet scheme? Primary Care 24 (PC24) are excited to share the position of Advanced Paediatric Nurse Practitioner (APNP).

This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint APNPs to support our 12 practices and our new purpose-built primary care hub offering a family-friendly service to our patients across Liverpool and Sefton.

We also have vacancies for face-to-face APNPs within our out of hours services with a variety of shift patterns.,

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.

  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.

  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.

  • Work with patients to support compliance with and adherence to prescribed treatments.

  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
