Advanced Nurse Practitioner

About the Role: As an ANP within Orpington PCN, you will work autonomously and collaboratively to assess, diagnose, treat, and manage patients with a range of conditions across our member practices. You will play a key role in supporting primary care services, reducing GP workload, and improving patient access and experience.,

• Conduct comprehensive clinical assessments and provide expert care to patients
• Diagnose and manage both acute and chronic conditions independently
• Prescribe medications within scope of practice and in line with local guidelines
• Contribute to health promotion, disease prevention and long-term condition management
• Work as part of a multidisciplinary team across GP practices within the PCN, Advanced Nurse Practitioners (ANPs) and paramedics delivering acute and complex care
• First Contact Physiotherapists (FCPs) supporting MSK assessment, diagnosis and management
• Podiatrists providing specialist foot care and chronic disease support
• Clinical Pharmacists and Pharmacy Technicians optimising medication, safety and long-term condition management
• Care Coordinators who support patient navigation, MDT coordination and proactive care
• Wider ARRS roles including social prescribing and mental health support
