Full Job Description

We are looking for an experienced Advanced Clinical Practitioner with a strong interest in frailty care to join our established multidisciplinary team delivering care within our community hospitals.

This role combines advanced clinical practice with clinical leadership; you will provide line management and professional supervision for the ACPs working within the service together with leading and coordinating the educational supervision of trainee ACPs.

You will provide advanced clinical care to patients with frailty, applying expert clinical reasoning and collaborative decision-making. You will contribute to the Trust-wide ACP strategy and frailty pathway development alongside supporting business planning, service redesign and efficiency initiatives.

Main duties of the job

The post holder will provide autonomous advanced clinical care to frail older adults admitted to community hospital wards across 6 community hospital sites. They will provide line management, professional leadership and clinical supervision for the Advanced Clinical Practitioners in the Community Hospitals.

The post holder will provide leadership and oversight to ensure Advanced Clinical Practitioners maintain professional standards and appropriate evidence for revalidation in line with national frameworks.

The post holder will lead and coordinate the educational supervision of trainee Advance Clinical Practitioners, supporting staff with educational programmes.

The post holder will lead or support frailty-related audits, service evaluation and quality improvement, using outcome data to improve patient safety and experience.
