Full Job Description
The Advanced Clinical Practitioner (ACP) will work as an integral member of the general practice multidisciplinary team, with a primary focus on acute, ontheday demand, alongside some longterm condition (LTC) management and home visits.,
- Assess, diagnose and manage patients presenting with acute problems on the day , within an agreed scope of practice and competence.
- Provide some planned reviews for stable longterm conditions and selected home visits, as agreed in the job plan.
- Work autonomously but with readily available GP supervision, escalating complex or highrisk cases appropriately.
- Help manage GP workload safely and effectively, while ensuring that GPs retain responsibility for undifferentiated, complex and highrisk presentations. This role is complementary to, and not a substitute for, GPs. GPs remain responsible for overall clinical leadership and holistic management of complex patients., We are also a training practice and are actively involved in supporting the development of future clinicians. We currently host two GP Registrars as well as undergraduate medical students, and the successful candidate will be expected to contribute to a supportive teaching and learning environment within the practice.
Talybont Surgery is a friendly, progressive, partner-led practice committed to delivering high-quality patient care within a supportive multidisciplinary environment. Our clinical team currently consists of six GPs (including four salaried and two partners), two GP Registrars, two Prescribing Pharmacists, a Mental Health Practitioner, a Womens Health Specialist Nurse, two Practice Nurses and a Healthcare Assistant (HCA). The clinical team is supported by an experienced and friendly administrative, reception and management team.
