Full Job Description
Adult Services sits within the People Directorate at West Northamptonshire Council and supports adults across a range of needs, including community support, learning disability services, hospital discharge, and transitions into adulthood. The focus is on enabling people to live independently, stay safe, and achieve the best possible outcomes. As an Adult Social Care Worker, you will be part of a supportive team delivering frontline social care. The role involves working directly with individuals, their families, and partner organisations to assess needs, develop support plans, and coordinate services. You will manage your own caseload, respond to changing circumstances including crisis situations, and ensure safeguarding responsibilities are met. You will be expected to work collaboratively across teams and services, maintaining accurate records and making informed decisions within a structured, legislative framework. The role also includes working flexibly across community settings and contributing to a consistent, person-centred approach across Adult Services.What will you be doing? As an Adult Social Care Worker, you will play a vital role in supporting vulnerable adults to live safe, independent, and fulfilling lives. You will take a person-centred, strengths-based approach, working alongside individuals, families, and professionals to co-produce meaningful support plans and improve outcomes. In this role, you will carry out person-centred assessments and co-create support plans, acting as a trusted assessor for defined areas of support. This includes conducting or supporting assessments and reviews (including risk assessments of both the individual and their carer), or facilitating self-assessments using appropriate tools to ensure plans are proportionate, cost-effective, and tailored to individual needs. You will manage a caseload, prioritising work effectively to support individuals, including those in crisis, helping to reduce risk and promote stability. Working in partnership with families, carers, and partner agencies, you will connect people to relevant community resources and services to promote independence and wellbeing. The role also requires you to identify and respond to safeguarding concerns, ensuring timely escalation where necessary, and to maintain clear, accurate, and up-to-date records using digital systems.

- Strong communication skills, including clear report writing
- Ability to build working relationships with individuals, families, and professionals
- Knowledge or experience of adult social care and key legislation (e.g. Care Act, Mental Capacity Act)
- Ability to manage workload, prioritise tasks, and work independently
- Able to assess risk and make decisions based on a risk-based approach
- Experience working in multi-agency environments
- Confident using IT systems (e.g. Office 365) and maintaining accurate records
- Ability to analyse situations and take appropriate action, including in challenging circumstances Desirable:
- Experience working with adults with learning disabilities
- Experience delivering social care outcomes using a strengths-based approach Other requirements:
- Relevant qualification (degree, NVQ Level 4 or equivalent experience)
- Ability to travel for work (driving required)
