Full Job Description

STARS is a short-term, intensive reablement programme that is available to help people to live at home following a stay in hospital, or if there has been a change in a person’s care and support needs. The aim of the service is to support people to regain independence, rebuild confidence and relearn skills that might have been lost due to illness.

Working Arrangements

Working Week: 37-hours per week - Monday to Friday. Shifts to be confirmed.

Expected Duration: 1 Year 3 Months
