Norfolk Community Health and Care offers exciting recruitment opportunities for an additional Outreach Team Leader based at our health led Children’s Centres in Poppylands North Norfolk. Norfolk Community Health & Care (NCH&C) requires your skills, enthusiasm and expertise to develop and support our Children’s Centres delivery.
• You will support the Integrated Children’s Centre Leader and help develop the outreach services for our Children’s Centres, where you will be pivotal in ensuring that services meet the requirements of the OFSTED framework for Children’s Centres and the Healthy Child Programme, and that services and activities are developed and evaluated to ensure improvement of outcomes for children.
• You will be responsible supporting the development of services to ensure parents have access to information,
advice on education, employment, training, family support, parenting and health services including the outreach
delivery of services.
• With a minimum degree level qualification in education, health, social care or community development, or actively
working towards this, you will lead and support staff to develop a range of activities and home based support that will improve outcomes for children and families.
