Marie Curie Nurse – Discharge

£30,831 per annum • Ipswich Hospital

Marie Curie Cancer Care provides free specialist support and care for cancer patients and their families. We aim to give patients the best possible quality of life, meeting all their medical, spiritual and social needs, whilst also providing their families with all the emotional support they need.

Join us in this challenging role and actively support discharge and help prevent inappropriate admissions into a busy district hospital, ensuring that care at home is a viable option for end of life patients and their families. You’ll be working in partnership with the local PCTs, NHS Providers, NHS Continuing Healthcare and the local hospice.

You will be a core member of the hospital and PCT specialist palliative care team and work daily with the discharge liaison team. Collaborating with other health and social care providers in primary, secondary, independent and voluntary care is essential. You’ll ensure communication links are in place to deliver a continuous, well-coordinated and seamless discharge process as well as tracking those patients appropriately admitted into the hospital. An effective advocate for the patient, you’ll understand the needs

of those receiving palliative/End of Life care as well as governance issues, risk management and audit procedures.

Possessing RN Level 1 registration, plus a relevant post registration qualification, this is your chance to influence change in a rewarding and highly valued role. Ref: SE979.
